Terms and Conditions
Clear Aligner are manufactured by CA Laboratory (known as the company in this
document) to the highest standards, the aligners are made from PVS impressions
(both upper and lower) taken by the dentist. It is the dentist’s responsibility to
assess the patient’s for orthodontic treatment, dentition, oral health and over all
suitability for treatment prior to submitting the appropriate Clear Aligner
prescription form. It is the prescribing dentist’s responsibility to retain and keep
all records in line with GDC recommendations. Clear Aligner will produce the
appropriate number of aligners depending on treatment requirements. Each set of
aligners are to be worn for one week 24/7 (minimum 20 hrs per day) except for
eating and cleaning in order to fulfil the treatment requirements. If additional
aligner are required, please contact us to discuss your requirements. Treatment
options are focused upon social six and limited movement for pre-molars.
Receipt and acceptance of the CA lab form constitutes and official and binding
order between the practitioner and CA laboratory Ltd
Clear Aligner system is based upon two phase putty and wash polyvinyl siloxane
(PVS) (we do not accept Impregum or alginate impressions) materials being used
(these impressions will be discarded after use unless the dentist stipulates their
return for which a service fee of £10 is charged). The accuracy of the impressions
will ensure correct fitting of the aligners. Any impression that have defects (air
bubbles or drags) maybe rejected prior to manufacture. If the impressions have
been rejected you will be notified by email and asked to resubmit new impressions.
The company cannot be held responsible for any additional costs that maybe
incurred either by the patient or practice when resubmitting new impressions. It is
the dentist’s responsibility to ensure all impressions are of the highest standards
and free from imperfections. All impressions submitted will not be returned but if
the dentist requires study models this should be indicated on the prescription
form and will be charged at the standard rate of £15 per set (these will be basic
models not Ortho trimmed). All impression should be free for contamination being
cleaned as per the appropriate disinfection guidelines.
The photos we need are – Open mouth pictures displaying both upper and lower
arch. Lateral left and right Buccal in Centric Occlusion, Full facial with lips
retracted in centric bite, full facial lips at rest and lateral profile lips at rest.
Clear Aligner has a published range of movements and will manufacture the aligners
according to the dentist’s prescription requirements and the overall assessment of
the patient dentition and oral health. Any remedial treatments for caries,
periodontal disease or TMD should be undertaken prior to treatment. The dentist
should observe these stipulated movement ranges any movements beyond those
published are the responsibility of the dentist. Particular attention should be paid
to rotations, intrusion and extrusions as these movements can be difficult to
Contradictions Clear Aligner case selection
Cases not suitable for Clear Aligner would be for instance:
Fixed bridge work, implants
Severe rotations beyond 30 degrees
Active periodontal disease
Poor Oral Hygiene
Extraction space closure
Un-erupted permanent teeth
Class III Orthodontic cases.
The cases which we provide Clear Aligners for are generally; mild, moderate
anterior misalignment cases, “some” lapsed Orthodontic cases maybe suitable for
Clear Aligner treatment. All cases treated by CA laboratories Ltd will require
digital treatment plan. This plan is subject to our standard change and requires
written approval from the practitioner for manufacture to proceed.
Patients Treatment (prior and during)
It is the dentist responsibility to ensure the patient both signs and understands
the terms and condition of the patient consent form by which the treatment is
undertaken. Inappropriate compliance, use or misdirection of use by the patient or
dentist may result in unsuccessful treatment. A case has deemed to be complete or
closed if a period of 12 weeks has lapsed without further written contact with CA
Laboratories regarding treatment. If the dentists decides to restart treatment
this will be subject to digital assessment and additional charges.
If the patient has not complied with the published wear time or product use the
company cannot be held responsible for incomplete treatments results. Should the
patient and the dentist decide to continue with the treatment new impressions and
treatment form will be required. This may be deemed by the company as a new
treatment and will be charged at the full published list price.
Patient treatments are for both adults and teens from the age of 14 upwards,
dentition should be fully erupted and not mixed.
Clear Align have a published price list which is updated regularly. We reserve the
right to amend, change, add, or discontinues any product published within our price
list at any time and without prior notice. This policy also applies to our published
price list. Lost or broken aligners will be charged as stated on our published price
Payment terms are strictly 28 days from date of invoice. The company reserves the right to set credit limits on individual or group accounts. If an account has not cleared of its outstanding invoices the company reserves the right to place the account on credit hold for which no further aligner manufacture will be undertaken. Dentist Finder and General Data Your practice data will be displayed on our public dentist finder this will notify enquiring patients that you are an approved provider of Clear Aligner. We reserve the right to remove your data from this site at any time and without prior notice. Conditions of removal are non-compliance to Clear Aligners terms and conditions, inappropriate use of Clear Aligner support materials and or collateral. Non- submission of a treatment cases within a six month period. Clear Aligner logos should be placed on the landing page of the practice web site. The position of your details on the dentist finder are determined by the company and the position may not reflect overall submission rates.
Please note all accounts beyond our credit terms will be passed to our debt collection agency, Sinclair Goldberg Price Ltd. All accounts, without exception, will be subject to a surcharge of 15% plus vat to cover our costs in recovery. These accounts will also be subject to any legal costs incurred in obtaining settlement.
Dentist Finder and General Data
You may list up to a maximum of two practices for which it is the dentist
responsibility to ensure the details of the practice are correct.
From time to time we will provide our carefully selected third party partners with
your details in order to further support the development of the Clear Aligner
brand within the practice. If you do not wish us to share this information we
should be notified in writing stating of your desire to be removed from either the
dentist finder or the third partner companies.
The company and employees or agents cannot be held liable for either the
treatment outcome patient injury or damage or if the product is used with other
appliances in order to facilitate different desired outcomes from those published
on the treatment form. The dentist is responsible for both the assessment and
treatment of the patient and the content of the submitted treatment form.
CA laboratories Ltd is a medical device manufacturer and does not practice
dentistry. The treating practitioner is solely responsible for prescribing and
administering orthodontic treatment. Clear aligner therapy is unpredictable. Clear
Aligner does not guarantee a successful treatment outcome. Individual results will
These terms and conditions are setup to ensure both patient and dentist have the
best experience when using the Clear Aligner product. Retention is also an
important part post treatment, if required we can provide the final stage aligners
to provide retention. However, retention should be monitored regularly to access
for any movements.
